Straight Answers For Agencies

Straightforward answers to the questions agencies ask most.

About the Service

What exactly is a pre-booking medical clearance?

A pre-booking medical clearance is an evaluation conducted to determine whether an arrestee is medically fit to be booked into a detention facility. Most jails require it before accepting custody of an individual who shows signs of intoxication, injury, or medical distress. Currently most agencies handle this by transporting the individual to an emergency department.

You can. But it’s expensive, slow, and pulls officers out of service for 2–6 hours per transport. ER staff aren’t trained for the custody environment and the documentation you receive isn’t designed for your intake workflow. Blue Line Medical solves all three of those problems.

Yes. Every Blue Line Medical provider is a licensed nurse practitioner or physician. We verify credentials before deployment and maintain documentation of provider qualifications for every encounter. Your agency will always know who responded and what their credentials are.

If our provider determines that an individual requires emergency medical treatment that cannot be handled on-site, we coordinate the appropriate referral. Our job is to handle what can be handled on-site and to make the right call when it can’t. We don’t cut corners on clinical judgment to avoid a transport.

Contracts and Cost

How much does it cost?

Pricing is structured based on your agency’s booking volume, geography, and coverage model. We don’t publish a flat rate because a rural sheriff’s office with 800 annual bookings has different needs than a county jail processing 15,000. Contact us and we’ll build a proposal around your actual numbers.

No minimum for the pilot program. Ongoing contracts are structured around your agency’s needs and procurement requirements. We don’t lock agencies into terms that don’t make sense for their situation.

Yes. Blue Line Medical contracts are structured as professional services agreements, which typically route through the same procurement and budget approval process as other contracted services. We can provide documentation to support your agency’s approval process.

Potentially. Depending on the grant and its allowable uses, contracted medical services for detention operations may qualify. We recommend checking with your grant administrator. We can provide service descriptions and cost documentation to support the application.

Operations

How fast can you respond to a clearance request?

Response time depends on your jurisdiction and the coverage model in place. Urban agencies with high volume typically have faster guaranteed response windows. Rural agencies may operate on a scheduled or on-call model. We’ll give you honest response time expectations for your specific location before you commit to anything.

We build coverage around your actual booking hours. If your facility processes bookings around the clock, we structure coverage to match. This is part of what we scope during the pilot and contract discussion.

Minimally. Blue Line Medical is designed to integrate into your existing workflow, not replace it. The main change is where the clearance happens — on-site instead of at the ER. Your booking staff continues doing what they do. We handle the medical piece.

Every encounter generates a standardized clearance record that includes the date, time, location, provider name and credentials, clinical findings, and clearance determination. Records are provided in a format compatible with your intake documentation. You get a complete paper trail on every encounter, every time.

You’ll receive a monthly clearance report showing total encounters, on-site clearance rate, ER transports, estimated cost avoidance versus an all-ER baseline, and officer hours recovered. Year-to-date totals accumulate each month, so the value compounds and stays visible to your administration throughout the year.

Yes. Blue Line Medical operates in compliance with applicable HIPAA requirements for covered entities and business associates. We can execute a Business Associate Agreement with your agency as part of the contracting process.
